Lore
Semi-automated these days, but someone still needs to load the shells in the racks. Years ago, there was a problem with the clamps that keep shells locked to the elevators and if there was any rattling that occured[sic] during operation, it would trigger this insane cascade failure where all the clamps would come loose one-by-one and send munitions rolling everywhere. It was even worse on a 450mm since that became a lethal game of 'dodge the falling artillery' with the grand prize of not having your head caved in by a 2,000 pound artillery shell.
Last I read, the Protectorate's OTDEL fixed it by just machining new bolts on the swing arms. I shouldn't say this but it was a very embarrassing situation for the Naval Office because they were adamant that it was crew negligence and opted to just have spacers watch more instructional videos.
